We welcome new patients who live within our practice boundary area to register with us. Patients who live outside of our practice boundary with a Derbyshire (DE) postcode are also able to register, but may be ineligible for certain services such as home visits or certain Derby city programmes and services.

Please register at the site closest to where you live, you will still be able to use services across all 4 Lister House sites regardless of which site you choose to register at. We ask that you register at the site closest to you to avoid issues with eligibility for accessing other external health services in the city. Please ensure that the information provided is accurate. If we identify a problem with your application or require additional information, the registration team will contact you to discuss this.
Anyone in England can register with a GP surgery. Registering at a GP practice is free of charge. You do not need proof of address or immigration status, ID or an NHS number. If you’re homeless, you can give a temporary address, such as a friend’s address, a day centre or the GP surgery address. You will need to present proof of identity if you ask for online access to medical records.

What are the acceptable examples of photo ID and proof of address?

Acceptable proof of address
- Recent utility bill (within 3 months of the current date)
- Local authority tax bill ( valid for current year)
- Bank or building society statement ( within 3 months of a current date)
- Current tenancy agreement
- Court order (within 12 months of the current date)
- UCAS letter (within 12 months of the current date)
What is temporary registration?
If you are ill while away from home or if you are not registered with a doctor but need to see one, you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

What if I live outside the practice area?
If you live or move outside of the practice area you are welcome to register or remain registered with Lister House Surgery (as long as you remain in a DE post code).
However, you will be considered an out of area patient and will not be eligible for certain services such as home visits or other Derby City services and programmes.
You should consider your health needs and whether it in your best interests or practical for you to be registered at Lister House Surgery. If your health needs change and you require more input from the health services, you may consider registering with a more local practice.
